Sacroiliac Joint Questionnaire

Summary

This invention consisting of two forms (one for the clinician and the one for the patient) captures the most commonly used signs and symptoms of sacroiliac joint pain, including symptomology, pain history, medical history, and social history.  
This invention also provides sections for reporting and evaluating previous treatments, radiographic and laboratory tests, and physical examinations related to the sacroiliac joint. This information can then be used as a validated way of diagnosing, studying, and reporting sacroiliac joint pain with high degree of accuracy and consistency. These forms also aid in the differential diagnosis of SIJ pain versus other forms of low back pain. They also aid in the differential diagnosis of referred “pseudoradiculopathy” in the leg due to SIJ pain from lumbar radiculopathy.
