Patient Tracker System

The Patient Tracker System is a web-based software tool that has been developed to allow for on-line workflow management and coordination of patient admission and discharge processes.  The system collects clinical criteria for discharge of a patient and provides a summary of discharge readiness status for the entire hospital facility, as well as occupancy status in real time. Other tools built into the system include generation of discharge orders, discharge instructions, and daily patient progress notes which can be incorporated into an electronic medical records database.  The Patient Tracker System also has decision logic tools for managing admission requests and identifies available resources such as surgical suites, patient rooms as well as other required resources.  The Patient Tracker System has been in use at Intermountain Healthcare and has been found to result in significant cost savings and improvements in the quality of patient care.  Published studies have shown overall improvement in the management of discharge process, reduction in delays in admission of patients and reduction in surgical procedure cancellations. The software is designed for use with Intermountain Healthcare's IT system, but can be adapted and integrated into other hospital IT systems.  Intermountain Healthcare is renowned for its pioneering use of advanced clinical information systems technologies to help healthcare personnel make better decisions and control costs.  This technology is available for license from Intermountain Healthcare.  For further information, please contact Mike Mayer, 801-408-1095, mike.mayer@imail.org.
